Provider Guide

New Mexico State law does require a referral from a licensed health care professional.   We graciously welcome all new referrals from providers.

In order to schedule your patient, we will need their referral and a demographic sheet faxed to us at 575-534-1439 (or the patient can call and schedule their own appointment).

We will then call the patient to set up their appointment and provide you with a plan of care after their initial evaluation.

If this is an emergency, please give us a call and we will see where we can fit them in.  We normally hold a few slots open during the week, just for emergencies.

If your patient will be visiting you during the course of their treatment, we also try and provide them with a progress report to give to you.  After we have finished the patients treatment, we will also send you their discharge report.

If at any time during the course of the patients treatment or if you have special requirements for your patent, please feel free to give our therapists a call. They are more than willing to work with you to get this patient well.

Occasionally, we can not get in contact with a patient due to a bad phone number, financial concerns like co-pays or the patient does not return our calls.  Please rest assured, that we do make every effort possible to schedule your patient for their visit; it is the lifeblood of our business.

If your patient never is scheduled or if they drop out of therapy during the course of their treatments, we will send you a letter explaining the situation.
