What To Expect Your First Visit
Please have your new patient forms completely filled out or show up 30 minutes early to complete them (you can also download them here).
Remember to bring your insurance cards and doctor’s prescription with you, if we don’t already have them.
Be prepared to undergo a complete physical exam on your first visit. It may take 45-60 minutes for the entire visit.
Quick Relief
We listen, understand and are quick in determining your needs. We offer treatments that, in most cases, brings immediate results and offer self-care tips for further recovery.
Be wary of the facility where you don’t learn something new each and every visit. Self-care and home exercise instruction are essential for fast results.
Don’t waste time in your therapy visits and run out of insurance benefits before your goals are met.
What To Wear
You may need to expose the body part that is affected (knee, shoulder, back, etc).
Loose fitting apparel such as sweats or a T-Shirt are ideal. Come a few minutes early if you will need to use our dressing room to change.
Your Appointments
It is our goal to help you get well as soon as possible. If you have any questions, or should you require our assistance in any way, please do not hesitate to call. We will respond to your needs as quickly as possible.
The best way to accomplish this is to keep your regularly scheduled appointments. Of course life intervenes periodically, however the majority of the time we would like to ask that you keep your scheduled appointment, or if you need to reschedule we will try to do so the same week.
We would also ask that you be prompt to your appointment to ensure quality care for you and all of our other patients. We apologize, but there may be times we won’t be able to fit you in even if you are only 5 minutes late.
Please remember to thank your doctor for referring you to our office. We hope that someday we are able to thank you for a referral as well.
Insurance Information
We accept Medicare and most major insurances.
We will need your (or your guarantor’s) insurance number and card to process your claims.
A co-pay or co-insurance may be due at the time of your visit. We accept cash, checks, visa, mastercard or discover cards. Depending upon how much your insurance pays for your treatments, there may be a small amount due or you may have a small refund coming after all of your insurance claims are processed (usually within 1 month after your final visit).
If you are a Medicare patient, there is a Medicare cap of $1740 per year for Physical Therapy Services. If you have seen a therapist during the last 12 months, please let us know, so we can watch your cap for you.
Workers Comp
To begin you workers compensation case, we will need to get in touch with your case worker before we can schedule your appointment.
Accidents and Private Insurance
If you have have been involved in an accident and have an attorney handling your case, we will need to get in touch with the attorney before we can schedule your appointment.
In some cases we also can file a lien against an insurance company other than your own who may be handling your case. Please give us the contact information of your insurance adjuster and we can proceed from there.
Self-Pay Patients
If you will be paying for treatments out of your own pocket, please let us know. Depending upon your circumstances, financial arrangements may be possible.
